Diagnostic Assessment with Dr. Tsutsumi $600 (120 minutes)
Therapy with Dr. Tsutsumi $350 (60 minutes)

Payment is due at the beginning of each session. Payments can be made by cash, check, or credit card (Visa, MasterCard, American Express, Discover).

Insurance Information

Dr. Tsutsumi does not accept insurance directly and is considered an “out of network” provider. Those with PPO insurance plans may get some of their appointments reimbursed if they have met their out of network deductible and their insurance allows out of network claims. If you have an HMO or Kaiser you typically will not receive an out of network benefit.

For those with PPO plans, you may be able to submit claims to your insurance provider for reimbursement when you submit your receipt to the insurance company along with a special out of network form that you request from your insurer. Dr. Tsutsumi will provide you with the necessary documentation, e.g., “super bill,” to aid you in this process but can make no guarantees regarding potential reimbursement.
